Summary of Health Benefits

Please note that all insurance and Flexible Spending Account eligibility begins on the 1st of the month after a thirty day waiting period.

 

MEDICAL COVERAGE/BASIC LIFE INSURANCE

Great West Health Insurance

Great West Health Insurance is our medical insurance provider.  Employees may elect to enroll in our standard High Choice Plan (Open Access Plus) or chose the Consumer Advantage Plan (PPO) which offers lower premiums in exchange for higher deductibles and co-insurance.  Employee only will automatically be enrolled in the High Choice Plan.  If dependents are enrolled in CAP plan, employee must be enrolled in CAP Plan as well. Through Great West, our employees are enrolled in a Prescription Plan and will also receive $20,000 worth of Group Term Life and AD&D.  Detailed Summary of Benefits Available upon request.

LONG-TERM DIABILITY

Sunlife Insurance
Long Term Disability

Bluware will pay 100% of this benefit for each employee.  All eligible Bluware employees must be enrolled in this Plan as it is 100% participation.  This benefit will pay 60% of your base monthly income if you qualify for Long Term Disability through Sunlife. With this coverage, if you become disabled, you can become entitled to 60% of your base monthly income. The elimination period is 90 days. During this 90 days, you must be completely disabled and receive no pay. After the 90 days are up, you become eligible to receive the 60% of your base monthly income.

Bluware provides Medical for all eligible employees.  However, since vision insurance is not included in this plan, employees may purchase vision insurance through CompBenefits.  If elected, premiums can be withheld from the employee’s paycheck on a pre-tax basis through the S125 Cafeteria Plan.   Detailed Summary of Benefits Available upon request.

HEALTHCARE REIMBURSEMENT ACCOUNT:  You may set aside tax-free dollars to pay for qualified medical, dental, vision or over the counter medications.   The maximum annual election for this account is $3,000.00.  A detailed list of allowable expenses is available upon request.  Receipts are required with each claim and direct deposit is available for a more timely reimbursement.
DEPENDENT CARE REIMBURSEMENT ACCOUNT:  You may set aside tax-free dollars to pay for qualified daycare expenses.  The maximum annual election for this account is $5000.00.  A detailed list of allowable expenses is available upon request.  Receipts are required with each claim and direct deposit is available for a more timely reimbursement.

 

BLUWARE, INC.’S 401(K) PROFIT SHARING PLAN

Bluware has established a 401(K) Profit Sharing Plan for eligible employees.  You are eligible to participate in the plan when:

  • Participants must be 21 years of age to qualify.
  • Participants must work ninety days of service and complete 500 hours worked in order to qualify.  Participant must be employed on 12/31 of the plan year in order to participate in Bluware Profit Sharing for that year.

Under the plan, you are able to set aside part of your earnings before Federal and State income taxes are withheld. Eligible employees may set aside 0-100% of their paycheck or any given dollar amount up to the IRS limits.  For more details or questions concerning the plan, please contact the Human Resources Department.
